Second Edition and First Edition Supplement

The Second Edition of our popular Model Forms for Child Care Agencies is now available. We are offering two options with this Second Edition. First, if you have not purchased the First Edition, we have a full Second Edition Publication bound in a three-ring binder and accompanied by a CD with all 56 Forms on the CD. Each form is a separate WORD file for you to personalize and edit to fit your agency. The second option is for providers who have purchased the First Edition. The First Edition Supplement includes 11 brand new forms and 15 edited/updated forms from the First Edition. This Supplement is meant to update your First Edition Model Forms publication with only the new and edited forms. The First Edition Supplement will be shipped with the 26 Forms and is accompanied by a CD. Each Form is a separate WORD file that can be edited to fit your specific agency needs.
